Important information!
The prescription is valid for 8 weeks from the date of its commencement. The Csokonyavisontai Spa must be indicated on the treatment form as the treatment location. In the case of spa treatment, the treatment must be used within 30 days of the date of the prescription. The first day of use of the treatment is the date of the first treatment. The patient must bring the treatment form with them to every treatment!
